That doesn't look plug and play, here's the pic of it with the required mods.
Don't forget to remove the two resistors near the switch as well as add two links and a wire, lol.

Hi

I have similar problem to earlier in a DI4001N double card reader it displays some leds with faded color
I know the BK for this and the key it is telling me is incorrect.

In another box DI4001NC it seems to work correctly.

I have just recieved it from tailor made.

Any Ideas ?

Just sounds like its not inserted properly, making proper contact.
