If you look around u can get these for half the price. £10 for the R4DS and £3 for the 2gb microsd card. I know coz i sell them and i sell lots of them.
The Genuine R4DS can only use a 2gb microsd card. The R4SDCH which is a bit more expensive can take upto an 8gb microsd card.
Also the R4 is an old developer card and no longer has a support team to update its firmware.

The SDHC versions yes can support bigger cards, however they have problems with booting sometimes, where you will have to restart your ds over and over.

I know this because I have used alot of version of these cards. The original is the most stable.
