HDMI (High-Definition Multimedia Interface) is a compact audio/video interface for transmitting uncompressed digital data.
I.E. its the connection that allows you to receive high-definition video

HD ready concerns the abilities of television receivers to display high-definition pictures.
1080p HD (1920x1080 progressive @24, 50 & 60 Hz)
1080i HD (1920x1080 interlaced @50 & 60 Hz)
720p HD (1280x720 progressive @50 & 60 Hz)
All above are different formats of HD display formats

Just to add to that, HD ready doesn't necessarily mean the T.V. will produce 1080p. HD ready reffers only to being able to show atleast 720p in a 16:9 format.

Also a DVI connection will produce a 1080p picture as will a component YPbPr connection.
